Job summary

Columbia River Mental Health Services is seeking a Housing Case Manager in Vancouver, WA. The role focuses on a Housing First approach, partnering with clients and landlords to secure and sustain housing, often starting from referrals from jail or community sources.

You will complete housing assessments, maintain EMR records, and coordinate with partners to provide long-term supports. Experience in social services and a Bachelor's degree are preferred.

Qualifications

• Approximately 2+ years of experience in the social service field with mental illness, medically fragile and/or chemical dependency preferred.
• Minimum of Bachelor's degree (or equivalent work experience) preferred.
• Strong commitment to recovery principles and client-centered care.
• Proficient computer skills with common office software and EMR systems.

Responsibilities

• Develop housing resources and assist consumers in obtaining and maintaining housing.
• Build consumer and landlord relationships and coordinate housing placements.
• Complete assessments and document in EMR; submit required monthly/quarterly reports.
• Coordinate with community partners for referrals and supported housing services.
• Attend staff and planning meetings; maintain accurate records and charts.
