Responsibilities

• Coordinate services to provide housing and prevent homelessness.
• Work with client, household members, supervisors and staff to execute the service plan.
• Establish goals addressing immediate housing crises and underlying causes.
• Monitor progress and evaluate progress of participants’ service plans.
• Prepare subsidy applications and necessary documentation for clients.
