Job summary

First Step House in Salt Lake City, UT, seeks a Permanent Supportive Housing Program Assistant/Case Manager who combines case management with program administration to help residents who have experienced homelessness and may face mental health or substance use challenges.

You’ll coordinate services, support housing stability, document in HMIS/EHR, and collaborate with clinical, housing, medical, and community partners, all within a 36-hour work week and a nonprofit-friendly benefits package.

Qualifications

• Bachelor's degree in Social Work, English, business, or related field preferred.
• Case management experience highly preferred.
• Experience with mental health or substance use treatment preferred.
• Valid Driver's License required.

Responsibilities

• Develop service plans with residents and help them work toward goals.
• Support residents in maintaining housing, benefits, healthcare, and stability.
• Coordinate services with outside providers and community resources.
• Maintain documentation in HMIS/EHR systems in a timely manner.
• Manage referrals, waitlists, intakes, and discharge processes.
• Assist with general administrative and program needs.
