Job summary

The Salvation Army St. Cloud Corps is seeking a dedicated Case Manager to provide individualized, person-centered services to shelter participants in St. Cloud, MN.

The role focuses on assessment, service planning, and supporting participants toward safe, stable housing and increased self-sufficiency. Essential duties include documentation, referrals, and coordinating with community partners, along with coaching on budgeting and employment readiness.

Qualifications

• Bachelor's degree in social work, human services, psychology, sociology, or a related field preferred. Equivalent education and experience may be considered.
• If the degree is in social work, Minnesota state licensure is required.
• One year of experience providing case management, social services, housing services, or related support services.
• Experience working with individuals and families experiencing homelessness or housing instability preferred.
• Must successfully pass background checks, possess a valid driver's license, and meet MVR clearance standards.

Responsibilities

• Provide person-centered case management to shelter participants. Assess needs, develop service plans, monitor progress, and address barriers to housing, income, employment, education, transportation, health care, and other needs.
• Maintain accurate, timely, confidential participant records; complete assessments, case notes, service plans, referrals, and reports per policies.
• Coordinate referrals and connections to community resources, service providers, landlords, and partner agencies; maintain community relationships.
• Provide coaching, guidance, and education on housing stability, budgeting, employment readiness, life skills, and related topics.
• Participate in team meetings, case consultations, and program activities; support safe, welcoming environment.
