Job summary

The Salvation Army St. Cloud Corps in Saint Cloud, MN is seeking a dedicated Case Manager to support shelter participants with individualized, housing-focused casework.

You will assess needs, develop service plans, coordinate resources, and guide participants toward stable housing and greater self-sufficiency. This full-time, on-site role requires one year of experience in related services; a bachelor's degree is preferred and Minnesota licensure if the degree is in social work.

Qualifications

Responsibilities

  • Case Management and Housing Stabilization (60%): Provide person-centered case management, complete assessments, develop service plans, monitor progress, and assist with housing, income, employment, education, transportation, health care, and other needs.
  • Documentation, Compliance, and Reporting (15%): Maintain accurate records, complete assessments, case notes, service plans, referrals, and reports per policies and funding requirements.
  • Resource Coordination and Community Partnerships (10%): Coordinate referrals and connections to community resources, service providers, landlords, and partner agencies; maintain relationships with community organizations.
  • Participant Education and Skill Development (10%): Provide coaching and educational opportunities on housing stability, budgeting, employment readiness, life skills; facilitate group activities.
  • Program Collaboration and Support (5%): Participate in team meetings, case consultations, program activities, seasonal initiatives, and events.

Program and Position Summary

The Salvation Army St. Cloud Corps is \"Doing the Most Good\" by providing holistic, person-centered services to individuals and families. In addition to worship services and pastoral care, services include a 69-bed shelter serving families, veterans, women, and men, emergency assistance, food programs, and connections to community resources.


The Case Manager provides individualized, person-centered case management services to shelter participants. Working collaboratively with individuals and families experiencing homelessness, the Case Manager assists participants in identifying strengths, addressing barriers to stability, connecting with community resources, and developing goals related to housing, income, employment, health, and overall well-being. Through assessment, advocacy, and ongoing support, the Case Manager helps participants move toward safe, stable housing and increased self-sufficiency.


$20.79 to $21.99 per hour, 40 hours per week

Essential Functions (approximate time spent)


  • Case Management and Housing Stabilization (60%): Provide person-centered case management services to shelter participants. Complete assessments, develop individualized service plans, monitor progress toward goals, and assist participants in addressing barriers related to housing, income, employment, education, transportation, health care, and other needs. Support participants in identifying and securing stable housing opportunities and community resources.

  • Documentation, Compliance, and Reporting (15%): Maintain accurate, timely, and confidential participant records in physical and electronic systems. Complete required assessments, case notes, service plans, referrals, and reports in accordance with agency, funding, and program requirements while ensuring compliance with applicable policies and procedures.

  • Resource Coordination and Community Partnerships (10%): Coordinate referrals and connections to community resources, service providers, landlords, and partner agencies. Maintain working relationships with community organizations to support participant needs and facilitate access to housing and supportive services.

  • Participant Education and Skill Development (10%): Provide coaching, guidance, and educational opportunities related to housing stability, budgeting, employment readiness, life skills, and other topics that support participant self-sufficiency and goal achievement. Facilitate individual and group-based educational activities as assigned.

  • Program Collaboration and Support (5%): Participate in team meetings, case consultations, program activities, seasonal initiatives, and special events. Collaborate with shelter staff to promote a safe, welcoming, and participant-centered environment and support overall program operations.


Education and Experience


  • Bachelor's degree in social work, human services, psychology, sociology, or a related field preferred. Equivalent education and experience may be considered.

  • If the degree is in social work, Minnesota state licensure is required.

  • One year of experience providing case management, social services, housing services, or related support services.

  • Experience working with individuals and families experiencing homelessness or housing instability preferred.

  • Must successfully pass background checks, possess a valid driver's license, and meet MVR clearance standards.

  • Occasional lifting and carrying of materials up to 20 pounds.

  • May encounter participants experiencing crisis, behavioral health concerns, or other barriers requiring de-escalation and problem-solving skills.

Driving/Travel Requirements


  • Local travel using an agency or personal vehicle may be required to attend meetings, trainings, participant appointments, housing-related activities, and community partner visits.
