Job summary

The Salvation Army St. Cloud Corps is seeking a Case Manager to provide individualized, person-centered services to shelter participants, helping them identify goals around housing, income, employment, and well-being.

You will assess, advocate, coordinate referrals, document progress, and support families and individuals toward safe, stable housing in a compassionate, community-focused environment.

Qualifications

• Bachelor's degree or equivalent in a related field is preferred.
• Licensure in Minnesota required if degree is in social work.
• Prior experience in case management or social services is preferred.

Responsibilities

• Provide person-centered case management to shelter participants.
• Complete assessments, service plans, and progress monitoring.
• Coordinate referrals to housing, health, and community resources.
• Educate and coach participants on housing stability and life skills.
• Participate in team meetings and program operations.
