Job summary

BRC is seeking a dedicated Case Manager to deliver comprehensive services for chronically homeless individuals. The role involves regular field and office meetings, thorough documentation, and progress tracking of housing plans.

You will collaborate with clinical staff to coordinate care and assist clients in overcoming barriers to housing, including entitlements. The position requires strong communication, bilingual Spanish in some cases, and CPR readiness.

Qualifications

• High School Diploma/GED required; bachelors degree preferred and masters ideal.
• Bilingual Spanish strongly preferred, but not required.
• Excellent communication and writing skills and computer proficiency required.
• Experience working in residential treatment and with the MICA and homeless populations.
• Knowledge of HRA and NY/NY application systems a plus.
• CPR training certification or willingness to take training class in CPR.

Responsibilities

• Provide comprehensive case management services to identified chronic homeless individuals.
• Meet with clients regularly in the field and in the office and ensure all clinical files and reports are up to date.
• Conduct comprehensive assessments to identify barriers to housing and assist clients to overcome them.
• Develop and progress on individualized housing plans and build a network of support services.
• Prepare clients for housing interviews, arrange housing appointments, and participate in case conferences as needed.
• Liaise with clinical staff in other programs regarding shared clients.
• Provide emergency first aid/CPR or overdose prevention protocol assistance when needed.
