Job summary

BRC is seeking a dedicated case manager in New York City to support chronically homeless individuals through intensive assessment, housing planning, and linkage to entitlements. You will meet clients regularly in both field and office settings, ensuring accurate records and coordinated care across the program continuum.

Responsibilities include developing individualized housing plans, preparing clients for housing interviews, and collaborating with clinical staff.

Qualifications

  • High School Diploma/GED required; bachelor’s degree preferred and master’s ideal.
  • Bilingual Spanish strongly preferred, but not required.
  • Excellent communication and writing skills and computer proficiency required.
  • Experience working in residential treatment and with the MICA and homeless populations.
  • Knowledge of HRA and NY/NY application systems a plus.
  • CPR training certification or willingness to take training class in CPR.

Responsibilities

  • Provide comprehensive case management services to identified chronic homeless individuals.
  • Meet with clients regularly in field and office; ensure clinical files and reports are complete.
  • Assess barriers to housing and help clients overcome them to secure entitlements.
  • Develop and work with clients on individualized housing plans and networks of support services.
  • Prepare clients for housing interviews and coordinate housing appointments.
  • Participate in case conferences and liaise with clinical staff as needed.
  • Provide emergency first aid/CPR or overdose protocol assistance when required.
