Job summary

CAMBA is seeking a dedicated Case Manager to support the HASA NY/NY III Scattered Site Housing Program in Brooklyn. You will handle contract-funded activities, document client services in Client Track, and connect clients to medical, mental health, legal, and housing resources.

This role requires a Bachelor’s Degree or an Associate/High School with 4+ years of relevant social-service experience. Full-time status with a comprehensive benefits package is offered.

Qualifications

• Bachelor’s Degree in a related field.
• Associates degree/high school diploma with at least 4 years’ experience in case management or related social-service field.

Responsibilities

• Provide all contracted activities described.
• Enter client services into Client Track (assessments/reassessments, service plans, progress notes).
• Refer and connect clients to needed services (medical, mental health, substance use treatment, legal aid, entitlements).
• Ensure clients are compliant with medical treatment and medication management adherence.
• Monitor clients’ progress toward goals via regular contact and visits; document progress notes.
• Act as liaison with outside organizations on education, healthcare, housing, legal issues, etc.
• Escort clients to appointments (educational, medical, social services).
• Assist clients in completing applications for benefits and entitlements; monitor outcomes.
