MACV is seeking an intensive case manager to serve Veterans and families who are homeless or at risk of homelessness in Moorhead and surrounding communities. The role requires conducting comprehensive assessments and developing housing stability plans in partnership with participants.
You will coordinate with local partners, provide mobile case management, track progress, and adjust plans to help Veterans achieve stable housing, economic security and well-being.

This position provides intensive case management services to Veterans and their families who are homeless or at risk of becoming homeless. The work of this position involves conducting client assessments (including financial, personal, and crisis) to identify issues/barriers and creating a housing stability plan with the Veteran to address the needs defined by the assessment.
This position will primarily serve Veterans and families in the Moorhead area, while also providing case management services to participants located in Bemidji and St. Cloud as needed. The position will coordinate closely with staff and community partners located in these areas to support participants, ensure continuity of services, and address housing stability needs. Occasional travel to these communities may be required based on participant and program needs.
