Job summary

MACV is seeking an intensive case manager to serve Veterans and families who are homeless or at risk of homelessness in Moorhead and surrounding communities. The role requires conducting comprehensive assessments and developing housing stability plans in partnership with participants.

You will coordinate with local partners, provide mobile case management, track progress, and adjust plans to help Veterans achieve stable housing, economic security and well-being.

Qualifications

• College degree in relevant field preferred.
• Experience delivering case management or direct services to homeless or housing-insecure individuals is preferred.
• Knowledge of veteran resources is a plus.

Responsibilities

• Perform intake with required documentation and program orientation.
• Develop housing stability plans with Veterans/families and update as needed.
• Provide mobile case management with a harm-reduction, strength-based approach.
• Coordinate with community partners to ensure access to services.
