Job summary

WestCare Foundation in Fresno, CA seeks a dedicated Case Manager to provide housing stability services for individuals and families referred to Homeless Prevention programs. You will coordinate services, conduct intake and assessments, maintain client files, and build partnerships with property owners to secure units for leasing.

Qualifications

• Excellent communication and public speaking skills.
• Ability to work with underserved populations.
• Ability to conduct outreach, including street outreach.
• Good computer skills and documentation capabilities.
• Strong Word/Excel proficiency; willing to learn.
• Ability to obtain and maintain background clearance and valid driver's license.

Responsibilities

• Oversight of day-to-day operations of the program.
• Builds and maintains relationships with property owners/managers to secure units for leasing.
• Meet with each client as needed and perform intake/assessment.
• Make referrals and appointments with appropriate agencies and case managers.
• Maintain client files and HMIS data entry.
• Provide daily, weekly, and monthly reports as requested.
• Coordinate services across several agencies and attend meetings.
