Job summary

A New Leaf is seeking a Tenancy Support Specialist in Phoenix to assist Seriously Mentally Ill individuals with housing stability under the H2O program. You will provide pre-tenancy and ongoing tenancy support, linking clients to community resources and services.

The role emphasizes accurate documentation, coordinated care with Health Homes, and advocacy for affordable housing options while ensuring compliance with program requirements.

Qualifications

• Minimum 6 months to 1 year of behavioral health experience.
• 6–12 months of experience supporting diverse homeless populations.
• Ability to manage a caseload effectively and provide individualized support.

Responsibilities

• Provide pre-tenancy and ongoing tenancy support to ~15 participants under H2O.
• Maintain minimum contacts weekly, with at least one in-person visit.
• Complete referrals, case notes, assessments, and reports per billing rules.
• Use HMIS/Unite Us/SharePoint for documentation and tracking.
