Position Summary:
The Case Manager provides comprehensive case management and supportive services to individuals and families participating in Community Services programs. The Case Manager works collaboratively with participants to assess needs, establish goals, connect them with appropriate resources, and support progress toward housing stability, self-sufficiency, and overall well-being.
This position is responsible for maintaining accurate participant records, coordinating services with community partners and other programs, and completing required program documentation and reporting. A successful candidate will demonstrate strong communication, organization, data entry, and customer service skills and be comfortable working with individuals and families from diverse backgrounds.

Responsibilities:
- Conduct participant intake, eligibility screening, and needs assessments in accordance with program requirements.
- Maintain an assigned caseload and provide ongoing case management and supportive services.
- Work collaboratively with participants to establish individualized goals and action plans that promote stability and self-sufficiency.
- Assist participants in identifying and securing safe, stable, and affordable housing, when applicable.
- Connect participants with appropriate community resources, including employment, education, childcare, healthcare, mental health, transportation, and other supportive services.
- Assist participants with budgeting, financial literacy, employment readiness, education goals, and other life skills as appropriate.
- Provide support related to activities of daily living and independent living skills, when applicable.
- Educate participants regarding leases, housing expectations, tenant responsibilities, and other relevant program requirements.
- Develop and maintain relationships with landlords, service providers, government agencies, and community partners.
- Conduct home, community, and office visits with participants as needed.
- Provide participant transportation when appropriate and authorized.
- Coordinate referrals and services with other UNC programs and external community partners.
- Maintain accurate and timely participant files, financial documentation, case notes, service records, and other required documentation.
- Enter and maintain participant and service information in ClientTrack and other required data management systems.
- Complete required program reports and assist with documentation necessary to meet grant, contract, and funder requirements.
- Maintain a thorough understanding of applicable program guidelines, eligibility requirements, policies, and funding requirements.
- Participate in community meetings, collaborative initiatives, trainings, and other activities relevant to assigned programs.
- Provide follow-up services to participants following program completion when required.
- Complete administrative and program-related responsibilities as assigned.
- Perform other duties as assigned by the Community Services Director.
Education/Experience:
- High school diploma or equivalent required.
- Bachelor's degree in social work, human services, counseling, psychology, sociology, or a related field is required.
- Previous experience in case management, human services, housing services, social services, or a related field preferred.
- Experience working with diverse populations and individuals experiencing barriers to housing, employment, or self-sufficiency preferred.
- Experience utilizing conflict resolution and de-escalation techniques in a human services, social services, behavioral health, housing, or similar setting required.
- Experience with electronic case management or client data systems preferred.

- Ability to maintain confidentiality and appropriately handle sensitive participant information.
- Proficiency with Microsoft Office 365, including Word, Outlook, and Excel, and the ability to learn client management and data-entry systems.
- Ability to maintain accurate and timely electronic and paper records.
