City of Aurora is seeking a Homeless Outreach Worker to build trust with unhoused neighbors, connect them to services and housing, and document progress in HMIS. You will coordinate with partners, conduct risk assessments, and help develop individual housing plans in a fast-paced field setting.

The role requires strong interpersonal skills, crisis intervention, and the ability to manage a 70+ caseload while navigating safety concerns and trauma-informed care.

Qualifications

• Experience working with unhoused or vulnerable populations.
• Strong communication and de-escalation skills.
• Ability to manage a caseload and coordinate with partners.

Responsabilités

• Build rapport with unhoused neighbors and assess immediate needs.
• Provide housing-focused case management and referrals.
• Enter data in HMIS and other systems promptly.
• Collaborate with mental health and substance use providers for referrals.
• Coordinate outreach, transportation, and resources for shelter access.
