Job summary

City of Aurora is seeking a Homeless Outreach Worker to build relationships with unhoused neighbors, act as a liaison to connect individuals and families to housing and supportive services, and work within Housing and Community Services to reduce homelessness.

The role involves trauma-informed engagement, coordination with partners, data entry in HMIS, and collaboration with abatement teams. Term-limited position with end date expected March 31, 2027.

Responsibilities

• Create and maintain rapport with unhoused individuals
• Engage individuals to assess needs (medical, mental health, shelter, benefits) to direct them to services
• Coordinate with community partners to link people to needed services for stabilization
• Participate in weekly meetings
• Reach out to encampments before and during abatements and offer resources and transportation to shelters
• Initiate safety plans and reduce harm for people experiencing homelessness
• Meet clients where they are with a trauma-informed approach
• Assist with abatement processes and provide outreach and basic needs
• Enter data in HMIS and other data systems accurately and timely
• Develop housing case plans with clients and monitor progress towards goals
• Provide crisis response and immediate support when needed
• Collaborate with abatement team and partner agencies
• Use City vehicles for outreach; may use personal vehicle as needed
• Coordinate with police and fire personnel on homelessness issues
• Provide resources and referrals for mental health and substance use providers
• Supply basic needs (socks, food, hygiene kits, weather-appropriate gear)
