Job summary

Familylinks in Western Pennsylvania seeks a Housing Coordinator for the Homeless Prevention Program to coordinate support services, assess eligibility, document requirements, and manage housing referrals.

You will conduct monthly face-to-face meetings, maintain case records, enter HMIS data, and collaborate with county funders. A bachelor’s degree is preferred and 2+ years in homeless services is desired; driving is required.

Qualifications

• Bachelor’s degree in social work or related field preferred.
• 2+ years’ homeless population experience preferred.
• Valid driver’s license and reliable transport.
• Flexible hours including evenings and weekends.
• MS Office (Outlook, Word, Excel) and HMIS experience.
• Excellent written and verbal communication skills.
• Self-directed with problem-solving abilities.

Responsibilities

• Accept referrals via Alleghenylink and review applications for eligibility.
• Collect required documentation from clients.
• Inspect apartments to meet county requirements.
• Submit client rental vouchers for reimbursement.
• Refer clients to job training, education, and housing options.
• Maintain case records and progress notes.
• Complete service plans and documentation.
• Advocate for clients to access services.
• Recertify eligibility annually.
• Meet clients monthly and document interactions.
• Coordinate with contract monitors and county funders.
• Enter monthly data in HMIS and report outcomes.
