Job summary

The Diversion Hub Housing Navigator is a Homeless Alliance employee embedded at the Diversion Hub in Oklahoma City, OK. This role provides home-based case management, field services, and resource referrals to clients experiencing homelessness to connect them with housing and stabilize them before discharge.

The Navigator works with Diversion Hub tenants and community partners to build wrap-around services, document care in HMIS and Salesforce, and support leases, entitlements, and HUD-related

Qualifications

• Undergraduate degree in social work or behavioural science is preferred.
• Two plus years of experience in adult case management.
• Computer literate; proficient with Microsoft Office (Word, Excel), Outlook, Internet.

Responsibilities

• Meets with clients at the Diversion Hub to discuss housing needs and options.
• Performs field-based case management using Housing First and related practices.
• Maintains documentation in HMIS and Sales Force per data standards.
• Assists clients with lease documents, IDs, and accessing benefits.
• Coordinates with Homeless Alliance and partners to build wrap-around services.
