Job summary

Responsibilities

• Promotes and supports individuals at risk of mental health or substance use issues with assessment, monitoring, linking to services, counseling, education, and advocacy.
• Provide services for Permanent Supportive Housing, focusing on helping clients maintain stable housing.
• Coordinate care with community resources and document progress and outcomes.
