Qualifications

• Bachelor’s Degree in social work/psychology or related human service field and one year of direct clinical experience, or RN license and at least one year of direct clinical experience.
• Candidate must meet QMHP guidelines.
• Effective problem solving and decision making skills.
• Ability to effectively intervene in a crisis situation.
• Good written and oral communication skills.
• Knowledge of local human service resources.
• Ability to use independent judgement and discretion in the assignment of duties.
• Good organizational and time management skills.
• Good computer skills and ability to effectively use electronic health record system.
• Ability to work flexible hours.
• Current valid driver’s license and good driving record.

Responsibilities

• Plan delivery of case management services to meet assessed needs.
• Link individuals to planned services and community resources.
• Coordinate services and plans with collateral agencies and providers.
• Monitor progress toward service plans and goals.
• Advocate for needed resources and services.
• Provide basic counseling services and home visits as needed.
• Explain psychiatric illness information and education to individuals.
• Complete administrative duties with emphasis on records and reports.
