Responsibilities

• Maintain a caseload of Housing Grant-funded clients, providing support services and prescribed activities to enable independence (home visits, case planning).
• Facilitate parenting classes with Housing Grant-funded families to improve communication and parenting skills.
• Back-up to Case Managers: maintain communication and case support when Case Managers are out of the office.
• Back-up to HCM: conduct budgeting classes with Housing Grant-funded clients to develop financial stability.
• When needed, conduct Grant Intakes with prospective clients (all Housing Grants).
• Learn each current Housing Grant and support the team in meeting grant expectations.
• Perform internal organization and grant file reviews to ensure compliance with FOS policies.
• Support HMIS compliance by reviewing data for accuracy and timely updates to client records.
• Execute monthly HMIS reports and address data deficits with staff.
• Audit Case Management files for compliance and readiness for reviews.
• Help organize data for grant reports and outcome tracking.
• Create and maintain library of Housing Client Success Stories for internal/external use.
• Keep visual trackers updated with client progress and goals.
• Help Case Managers track deadlines to ensure policy compliance.
• Work with the team to ensure follow-through on data deficits and KPIs.
• Review SharePoint documents to ensure access to up-to-date documentation.
