Job summary

Avalon Housing in Ann Arbor, MI, seeks a Miller Manor Support Coordinator (Case Manager) to perform outreach, assessments, care coordination, and housing-support services in a 24/7 staffed setting. You will work with tenants facing health, behavioral, and housing challenges to promote stability and independence.

Qualifications

• Bachelor's or Master's in a social/behavioral science or relevant field; or three years of relevant experience.
• Experience working with homeless populations, mental illness, chronic health, and/or addiction disorders.
• Ability to engage diverse populations in a non-judgmental way and respond to crises.
• Flexibility to work occasional evenings and weekends; strong teamwork.

Responsibilities

• Provide ongoing outreach and creative engagement to tenants.
• Conduct comprehensive assessments and help tenants develop action plans to achieve goals.
• Coordinate eviction prevention efforts and housing permanency plans with property management.
• Assist tenants with basic life skills, budgeting, and responsibility for their apartments.
• Link tenants to employment opportunities and skill development.
• Provide access to medical, mental health, substance use, and psychosocial supports.
• Assist with conflict resolution among tenants and neighbors.
• Provide crisis intervention as needed and document activities per program requirements.
