Job summary

Community Human Services is seeking a Community Support Specialist for Bridge Housing to provide case management and supportive services in our residential programs. The role emphasizes Housing First and Harm Reduction, collaboration with staff, and timely documentation.

You will build relationships with residents, assist with goals and benefits access, and help plan life skills workshops while ensuring safety in a 24/7 facility.

Qualifications

• High school diploma plus two years’ work experience in case management or equivalent, as approved by the agency/funder.
• Act 34, Act 33, and FBI fingerprint clearances required upon hire.

Responsibilities

• Develop a professional and therapeutic relationship with residents.
• Maintain appropriate personal boundaries in line with CHS professionalism policy.
• Provide direct case management to residents including goal planning and benefits access.
• Perform safety and security checks inside and outside the facility as required.
• Clean staff office, showers, common spaces, and prepare rooms for new residents.
• Distribute pre-packaged meals following health and safety regulations.
• Follow communication protocols and promptly address complaints or behavioral issues.
• Collaborate with case management team to develop individualized housing and employment goals.
• Assist with life skills workshops and training on budgeting, meal planning, and tenant rights.
• Facilitate connections to community resources, volunteering, and employment services.
• Complete daily written documentation including progress notes and goal updates.
