Job summary

Step Up is seeking a Fulton County Permanent Supportive Housing Case Manager to provide engagement and housing stability support as part of a Housing First team serving chronically homeless adults in the Metro Atlanta area.

You will conduct intake and assessments, develop service plans, coordinate with clinical resources, assist with moving into housing, and support vocational goals while maintaining HMIS documentation.

Qualifications

• Bachelor's Degree required if not a Certified Peer Specialist.
• Certified Peer Specialist require a High School Diploma.
• Minimum 2 years housing/case management experience.
• Proficient use of Office 365 products.

Responsibilities

• Provide intensive case management Housing First services to chronically homeless persons to maintain Permanent Supportive Housing.
• Meets with assigned members individually through home visits and/or telehealth sessions to review progress toward goals.
• Coordinate with community treatment providers to ensure ongoing clinical supports and successful transition to and maintenance of independent living.
• Support members' vocational choices and assist with overcoming job-related anxiety and choosing positions that match their strengths.
• Assist members in building social and interpersonal skills to enhance employment opportunities and job retention.
• Monitor warning signs of de-compensation and coordinate with the treatment team to respond accordingly.
• Attend staff meetings, supervision, and training as required.
