Job summary

LifeWorks of Austin is seeking a Case Manager to join our Permanent Supportive Housing team. This on-site, full-time role provides housing navigation, community-based case management, and data entry to support clients in securing stable housing.

Responsibilities include client-centered service planning, coordination with health partners, and ensuring documentation meets program and funder requirements. Benefits are included with PTO, holidays, and retirement options.

Qualifications

• Requires a Bachelor’s Degree in social or behavior sciences or administrative field.
• Minimum 2 years related experience; or an equivalent combination.
• Lived experience of homelessness is a plus and may count towards qualifications.
• Proficient in HMIS, Microsoft Office, Outlook, and agency database.

Responsibilities

• Provide client-centered, healing-centered case management services to diverse individuals experiencing homelessness or related challenges.
• Identify and leverage clients’ strengths as part of service planning to help clients achieve their goals.
• Work with teammates to ensure fidelity of the Strengths Model of Case Management and documentation.
• Assist clients with day-to-day needs and development of life skills (e.g., budgeting, transportation, job search).
• Assess clients’ wellness needs and develop tailored wraparound supports and referrals.
• Manage incidents and crises with de-escalation and safety planning techniques.
• Engage with community partners to advocate for client needs and coordinate services.
• Help clients build and reconnect social support networks.
