Job summary

Healing Transitions in Raleigh seeks a Transition Case Manager to support Alumni and CTR II participants with employment, housing stability, and financial self-sufficiency. Qualifications

• 2+ years in recovery or related field preferred.
• Must have valid driver license and transportation.
• HS diploma or equivalent preferred.
• 1+ year of relevant experience or equivalent education.
• Ability to define problems, collect data, and draw conclusions.
• Must pass negative pre-employment drug test.

Responsibilities

• Coordinate CTR II team meetings and support communication regarding participant needs, progress, and program activities.
• Work with other departments to address issues and coordinate services for participants.
• Communicate participant needs, barriers, progress, and outcomes while maintaining confidentiality.
• Support program activities and organizational initiatives.
• Review housing applications for Women’s and Children’s Housing and determine eligibility.
• Maintain the application pool for interested residents.
• Assist CTR II participants in securing employment and housing.
• Coordinate and track resident rental payments.
• Maintain rent deposits and rent collection for CTR II participants.
• Provide case management and continuing support for Job’s Journey tenants.
• Provide referrals, ongoing alumni support, and act as point person for Retrack.
