Job summary

Methodist Home for Children in Edenton, NC is hiring a Transitional Living Specialist to support male youth in a structured living program. You will lead case management, set personalized goals, connect residents with education, jobs, and housing resources, and collaborate with families and community partners to ensure safe transitions.

The role emphasizes mentorship, independent living skills, and ongoing aftercare for up to six months to help residents achieve lasting progress.

Qualifications

• Bachelor's degree required in Human Services, Social Work, Psychology, Criminal Justice, Sociology, Education, or related field.
• At least two years of relevant experience with youth or similar population.
• Licensed driver with a clean driving record.

Responsibilities

• Provide comprehensive case management and individualized support to residents.
• Develop and monitor goals related to education, employment, independent living, and successful transition to adulthood.
• Help residents identify and pursue education, career, and vocational opportunities.
• Coordinate Child and Family Team (CFT) meetings and collaborate with families, probation staff, schools, employers, service providers, and other community partners.
• Maintain accurate documentation and monitor each resident's progress.
• Develop individualized transition plans to prepare residents for successful discharge.
• Connect residents with community resources, employment opportunities, educational programs, and other supports.
• Serve as a consistent mentor and positive role model while helping youth develop confidence, responsibility, and independence.
• Provide aftercare services for up to six months following placement to help residents maintain progress and successfully transition into the community.
