Job summary

Optima Medical Management Group seeks a Community Support Outreach Specialist to assess members, develop housing plans, and connect them with resources to end homelessness. You will collaborate with service teams and program partners to tailor support plans and track outcomes.

The role requires strong communication, case management experience, and the ability to travel in the Sacramento area. A valid CA driver's license and vehicle insurance are essential.

Qualifications

• High school diploma or GED required.
• Minimum 1 year experience in case management, patient care, customer service, call center, or patient care.
• Valid California driver’s license and vehicle insurance.
• Excellent communication and organizational skills.
• Ability to travel using personal vehicle.

Responsibilities

• Screen and assess members and families to gather health, housing, and income information.
• Develop individualized housing support plans based on assessments.
• Maintain communication with teams, landlords, and community partners.
• Provide life skills, job readiness, and financial literacy coaching.
• Keep member records and report progress to Supervisor.
