Job summary

Optima Medical Management Group is seeking a Street Outreach Supervisor to lead outreach efforts for ECM and CS programs in the San Diego area. You will supervise a team, establish community relationships, and drive enrollment targets while coordinating with healthcare partners.

The role requires leadership experience, 2+ years in outreach, and a focus on vulnerable populations, with bilingual English/Spanish preferred and HIPAA awareness. Competitive benefits apply.

Qualifications

• 2+ years in community outreach or healthcare program coordination.
• Previous supervisory or leadership experience.
• Experience with ECM/CS and CalAIM preferred.
• Experience working with vulnerable populations.
• Strong understanding of social determinants of health.

Responsibilities

• Identify and establish relationships with key community locations, including shelters, health centers, social service agencies, libraries, and food banks.
• Develop and maintain a comprehensive mapping system of outreach locations and resources.
• Build partnerships with community-based organizations and healthcare providers.
• Facilitate enrollment processes and ensure proper documentation and consent procedures.
• Meet daily, weekly and monthly enrollment targets and analyze outreach data.
