Job summary

Community Bridges, Inc. is seeking an Associate Director of Housing and Community Integration in Phoenix to oversee outreach, shelter, housing and community-based programs. You will supervise managers, ensure contract compliance, and drive outcomes with staff development and training plans.

The role partners with the finance team on grants and budgets while expanding community partnerships and funding opportunities to grow the continuum of care for patients and communities served by CBI.

Qualifications

• Must have 1 year in a behavioral health position providing outreach and engagement activities.
• 1–3 years of grant writing experience, program development and/or grant monitoring.

Responsibilities

• Oversee outreach, shelter, housing and community-based programs in the defined area.
• Supervise management staff over these programs.
• Ensure programs meet contract requirements, productivity targets and outcomes.
• Develop training plans to address gaps in care and staff development.
• Collaborate with finance on grant management and budget tracking; monitor for compliance.
• Represent Community Bridges in the community and seek partnerships and funding opportunities.
• Serve as liaison for CBI Patient Centered Medical Homes, Crisis, and Inpatient units to Housing resources.
