Job summary

Community Bridges, Inc. (CBI) is seeking a Client Care Coordinator for our Phoenix, AZ site to manage a housing caseload and coordinate care for individuals and families experiencing homelessness. The role ensures connection to PCMH/ PCP services and coordinates RBHA and community-based services.

You will record care coordination activities, contribute to program reports, and work with a multidisciplinary team to improve outcomes for high-acuity clients.

Qualifications

• High school diploma or GED required; associate degree preferred.
• Minimum 12 months recovery from substance use and/or mental health disorders.
• 6 months of paid related work experience in behavioral/medical crisis exposure required.
• 1–3 years health care related work experience preferred.

Responsibilities

• Coordinate care for individuals and families experiencing homelessness at shelter/temporary lodging sites.
• Link clients to housing, medical, behavioral health services and PCMH/ PCP care.
• Document coordinated care and produce end-of-shift reports; assist with program data entry.
