Catholic Charities DC is seeking a Case Manager to provide direct management services, coordinating community resources and supporting clients with short- and long-term goals. The role involves collaborating with a multidisciplinary team, performing home visits for housing navigation, and maintaining comprehensive case records.

The position requires a Bachelor’s degree in human services, licensure in social work or professional counseling, and prior experience with at-risk or homeless

Qualifikationen

• Bachelor’s degree in human services or related field.
• Social work licensure in the appropriate jurisdiction is required.
• Professional counseling licensure in the appropriate jurisdiction is required.

Aufgaben

• Interview clients for direct service needs and develop a case plan with short- and long-term goals.
• Reevaluate goals with clients at regular intervals and update plans accordingly.
• Facilitate individual and group meetings and collaborate with a multidisciplinary team.
• Make appropriate referrals for emotional, substance abuse, legal, medical, dental, educational, vocational, employment, and housing needs.
• Create and maintain case records including assessments, service updates, and reports.
• Enter DAP notes within 48 hours into the Homeless to Housing portal.
