Safe Berks is seeking a Housing Case Manager in Reading, PA to provide individualized advocacy and support for survivors of domestic violence and housing instability. You will meet participants monthly to develop service plans, help secure housing options, employment and benefits, and monitor progress.

The role requires trauma-informed service delivery, strong communication, and thorough recordkeeping in HMIS/ET0, with collaboration across housing and coordinated-entry teams.

Qualifications

• Bachelor’s degree in Social Work, Psychology, or related field preferred.
• High school diploma required.
• Experience in direct service is a plus.

Responsabilités

• Provide individualized support and advocacy to participants in Safe Berks’ Rapid Rehousing Program.
• Meet with participants monthly to develop and monitor service plans.
• Assist participants with securing housing, employment, and benefits.
• Maintain confidential participant records and HMIS/ETO entries.
• Coordinate with the BCEH Coordinated Entry Manager on referrals.
• Attend required staff and coalition meetings.
