Job summary

Buffalo Federation of Neighborhood Centers is hiring a SHP Housing Case Manager in Buffalo, NY to help individuals with mental health diagnoses obtain safe housing and maintain residential stability. You will coordinate housing options, moves, and services with other providers to determine eligibility for housing programs and resolve landlord/tenant issues.

Responsibilities include developing daily living skills, reviewing client rights, and maintaining timely documentation.

Qualifications

• High School Diploma and five years full time work experience in human services.
• One year paid experience in direct service to persons with disabilities, residential setting preferred.
• Computer literate (MS Word, Excel, Outlook) and valid driver's license with good driving record.
• Ability to work some evenings and weekends.

Responsibilities

• Provides advocacy and linkage to behavioral health, medical and community services.
• Engage program participants to develop rapport and build trust.
• Locate rental units and negotiate leases within program budget and guidelines.
• Set up apartments and help clients acclimate to new environment.
• Maintain case records and complete required documentation per agency guidelines.
• Prepare and submit monthly/quarterly/annual reports on a timely basis.
