Job summary

Qualifications

• Associate degree with 1 year qualifying experience.
• High School Diploma with 2 years qualifying experience.
• Must possess a valid driver’s license and own vehicle for job requirements.

Responsibilities

• Exhibit agency core values in daily activities.
• Assist clients in locating safe, affordable housing in preferred area.
• Support clients through program phases and goal setting for sustainable housing.
• Purchase household furnishings for participant residence.
• Assist in obtaining benefits, income collection, and rental stipends if needed.
• Maintain communication with treatment providers, families, landlords, and supports.
• Record progress notes in Millennium Database and complete required paperwork.
• Attend team meetings in the community and perform other duties as assigned.
