Job summary

CAMBA, a community service organization in New York City, seeks a Housing Case Manager for the Short Term Housing Program in Brooklyn. You will conduct eligibility, complete health and housing assessments, and coordinate services to connect clients with housing and primary HIV medical care.

Qualifications

• Two years of relevant experience in case management or related social services.
• Good written and verbal communication.
• Computer literacy in Microsoft Office.
• Bilingual in Spanish is a must.

Responsibilities

• Conduct prescreening to determine program eligibility and gather verification documents.
• Enter client services into eSHARE and maintain audit-ready charts.
• Connect clients to medical, mental health, legal, and entitlements services.
• Assist clients in completing applications for benefits and entitlements.
• Escort clients to appointments and document progress notes.
• Monitor progress and meet monthly projections and program reports.
