Job summary

Access Services in Bucks County, PA, invites a compassionate Housing Case Manager to support individuals in the SAIL Housing program. You will conduct regular home visits, coordinate with landlords and providers, and help participants achieve housing stability and independence.

Qualifications

• Bachelor’s degree in a human services-related field is required.
• One year of relevant experience in affordable housing or human services is required.
• Experience in Bucks County housing system is strongly preferred.

Responsibilities

• Provide community-based housing support through regular home visits and help participants build independent living skills.
• Develop individualized goals with participants to promote housing stability and well-being.
• Coordinate connections to community resources, benefits, healthcare, and employment services.
• Support participants through the housing process, including lease signings and tenancy preparation.
• Serve as liaison among participants, landlords, treatment providers, and partners to improve outcomes.
• Identify barriers to stability and connect to additional supports or higher care when needed.
• Maintain accurate documentation and participate in program reviews.
