Access Services in Bucks County, PA is hiring a Housing Case Manager for the SAIL Housing program to support individuals seeking housing stability and independence. You will conduct regular home visits, coordinate services, and collaborate with landlords and community partners to promote successful independent living.

Qualifications

• Bachelor’s degree in a human services field required.
• One year of relevant experience in affordable housing or human services required.
• Experience in Bucks County housing system strongly preferred.
• Experience with vulnerable populations preferred.

Responsibilities

• Provide community-based housing support through regular home visits, helping participants build independent living skills and maintain stable housing.
• Partner with participants to develop individualized goals that promote housing stability, self-sufficiency, and well-being.
• Coordinate and connect participants to community resources, benefits, healthcare, employment services, and other supports.
• Support participants during lease signings, housing transitions, transportation assistance, and preparation for tenancy.
• Liaise between participants, landlords, treatment providers, and community partners to promote collaboration and positive housing outcomes.
• Identify and address barriers to housing stability through advocacy, problem-solving, and coordination of additional supports.
• Maintain timely and accurate documentation, records, and reporting; participate in case reviews.
