Job summary

St. Luke’s University Health Network seeks a Housing Specialist to support ACT participants in securing and maintaining safe, affordable housing within Bucks County and surrounding communities.

You will collaborate with landlords, assist with funding applications, and coordinate housing resources to promote stability. The role requires a Bachelor’s degree in a related field, at least two years of direct-service experience with mental health or forensic populations, and a valid driver’s license

Qualifications

• English fluency required in speech, reading, and writing.
• Mathematical reasoning: basic.
• Clean driving record and active driver’s license required.

Responsibilities

• Meet with ACT participants—in their homes and other community venues—to provide in vivo services that improve quality of life.
• Provide medication service—including delivery of medications and observation of ingestion—as directed by the team psychiatrist and staff.
• Coordinate services for ACT participants by developing, implementing and evaluating treatment goals; provide individual counseling.
• Provide on-call crisis services on a rotating schedule to cover program needs.
• Complete documentation in the electronic health record per standards; work weekends, evenings, and holidays as needed.
• Crosstrain with ACT specialties to provide additional services (e.g., forensic, vocational support).
• Build and maintain relationships with landlords and housing stakeholders to develop housing resources.
• Assist participants with accessing and maintaining housing and housing-related funding.
