Job summary

ADP is seeking a Housing Case Manager in Richmond, VA to deliver field and clinic-based case management for housing program participants affected by substance use. The role involves assessing client needs, creating personalized housing plans, and coordinating services with landlords and providers to promote housing stability.

The position emphasizes compassionate, client-centered support, rapid intake, and collaboration with diverse partners to navigate housing systems and access resources.

Qualifications

• Minimum one year of experience in recovery work, case management, social services, or housing support.
• Knowledge of housing resources, public assistance programs, and community service networks.
• Knowledge of chemical dependency, mental health, and housing/homeless service systems.
• Strong interpersonal and communication skills to engage diverse clients and stakeholders.
• Ability to manage multiple cases while meeting deadlines.

Responsibilities

• Conduct housing-focused outreach with community providers to individuals impacted by substance use.
• Complete housing assessments, stabilization plans, and eligibility documentation with Housing Navigator.
• Provide ongoing field and clinic case management to support housing retention and budgeting.
• Coordinate patient care with prescribers, clinicians, peer recovery specialists, medical providers, and agencies.
• Advocate for clients on eviction prevention, lease negotiations, and access to financial assistance.
• Maintain accurate documentation of client interactions and outcomes per requirements.
• Participate in multidisciplinary meetings and contribute to program improvements.
