The Fortune Society, Inc. is seeking a Case Manager (JISH) to perform initial assessments, provide ongoing supportive services in the field and office, and maintain documentation for justice-involved clients with histories of mental illness, substance use and homelessness.
The role requires travel across the five boroughs via mass transit, strong organizational skills, and the ability to coordinate with housing providers, benefits agencies, and program staff.

Reports to: Program Director
Status: Full Time, Regular, Non Exempt
Salary Range: $32.97 to $33.71 per hour (approximately $60,000 to $65,000 annually)
Location: Harlem, New York
Days/Hours: Monday to Friday, 9am to 5pm (some flexibility required)
Position Summary: The Case Manager will perform initial assessments, provide ongoing supportive services both in the field and in the office and maintain documentation requirements for justice involved clients with histories of mental illness, substance use and homelessness.
Education: Associates Degree in Human Services or/and five years of experience working with special needs population; a High School Diploma and eight years of experience working with special population or a bachelors degree in human services is preferred with at least 2 years of experience with the target population.
Work Experience: Candidate must have the ability to oversee a caseload of 15 clients. In addition, the candidate must be able to provide a collaborative assessment of needs, create and implement service plan, monitor progress and evaluate services provided to the individual client.
Computer Skills: Proficient in Microsoft Excel, Word, and Outlook with a working knowledge of PowerPoint.
Communication Skills: Excellent written and verbal communication skills.
General Work Behaviors/Skills: Excellent interpersonal skills with ability to interact culturally, linguistically, and diplomatically with diverse internal and external individuals.
