Job summary

Taleo in New York seeks a Case Manager to guide clients toward housing permanency by coordinating health care, job training, housing, and immigration services. You will empower clients through empathy, accountability, and trust while developing collaborative service plans.

You will interview 25 clients, develop individual plans with short and long-term goals, monitor progress, and advocate with agencies to remove barriers to services and re-housing.

Qualifications

• Bachelor’s degree in human services or related field with relevant experience.
• Sensitivity to disenfranchised communities including LGBTQ+ and homelessness.
• Experience with homeless population preferred; strong written and oral communication.
• Ability to respond to crises, work under pressure, and handle flexible hours.

Responsibilities

• Carry a caseload of clients and interview them within 24 hours of admission.
• Develop individualized service plans with short- and long-term goals.
• Coordinate referrals for health care, job training, housing, and immigration services.
• Monitor progress with weekly appointments and bi-weekly reviews.
• Advocate with agencies, accompany clients to appointments, and assist with re-housing.
• Maintain orderly client files and enforce program rules with shelter staff.
