Job summary

Foundation Communities Inc. is seeking a Case Manager for the Sierra Ridge community in Austin, TX. This full-time role manages a caseload of families in the Children’s HOME Initiative, coordinating services with property management and resident services teams to promote housing stability and program outcomes.

The position emphasizes trauma-responsive services, collaboration with a cross-functional team, and direct support to extremely low-income families over a 24-month period.

Qualifications

• Bachelor’s degree in social work, counseling, psychology, or related field.
• Minimum of two years of trauma-responsive services experience with ethnically diverse or unhoused populations.
• Proven ability to work independently and in a multidisciplinary team.

Responsibilities

• Manage a caseload of families in the program and provide case management and referrals.
• Meet regularly with families to identify needs and set attainable goals and track progress.
• Advocate for client needs and provide crisis intervention when needed.
• Coordinate access to resources and services from partner providers.
• Assist with educational opportunities to improve skills and stability.
• Maintain compliant documentation for all participant families.
• Support resident councils and community outreach activities.
