Job summary

Dignity Housing seeks a Case Manager for the Enhanced Service Project (ESP) in Philadelphia. You will provide case management to single adults and families to sustain housing and connect them with supportive services.

You will work in a team, developing housing and service plans and referrals, serving residents in scattered sites from the Germantown office. The role requires at least a bachelor’s in a Human Services field and 2+ years of relevant experience, with some evenings required.

Qualifications

• Bachelor’s degree in a relevant Human Services field is required.
• Minimum of two years experience providing counseling and case management for homeless or related populations.
• Ability to work across multiple sites with flexible hours including some evenings.

Responsibilities

• Conduct monthly home site visits and complete service plans.
• Assist residents with medical access, benefits, transportation, and utility support.
• Document contacts, updates, and referrals in HMIS and client charts.
