Western New York Integrated Care Collaborative, Inc. seeks a dedicated case manager to oversee HUD Scattered Site Housing, OASAS PSH, Shelter Plus Care, and Garden Lofts programs, coordinating housing placement and supports for individuals experiencing homelessness with substance use disorders in Buffalo.

Qualifications

• Master’s Degree in Human Services or related field is required.
• Bachelor’s Degree in Human Services or related field with at least 6 months of experience.
• Associate’s Degree in Human Services or related field with at least 1 year of experience.
• Valid New York State driver’s license and a reliable automobile are required.

Responsabilités

• Assists program participants in obtaining housing that meets HUD standards.
• Develops individual service plans to support growth, empowerment, and self-sufficiency.
• Maintains housing charts and regulatory documentation.
• Attends weekly staff meetings and case conferences.
• Responds to crises and emergencies as needed.
• Maintains confidentiality and complies with HIPAA, 42 CFR Part 2, HUD rules.
