Role Description

This is a full-time hybrid role for a Housing Case Manager at Safeway Home Healthcare located in Roseville, MN. The Housing Case Manager will be responsible for assisting clients with finding and maintaining safe and stable housing, coordinating resources, conducting assessments, and developing individualized housing plans. Some work from home is acceptable in this role.
