Opportunity Community Services is looking for a passionate Housing Case Manager that would enjoy some occasional long-distance driving (60+ miles) in conjunction with clients in the Metro area. Work experience in addition and dependency is strongly preferred. The average caseload of long-distance clients for this position is 3-4 clients. OCS supports clients all over the state of Minnesota. Some perks for staff who have long-distance clients enjoy: an increase in mileage reimbursement, paid hotel stays (clients who are over 150+ miles), meal reimbursement and the next day flex schedule!
Compensation:
- Starting pay: $21.00-$22.00 per hour
- Standard Salary Range: $21.00-$25.00 per hour
- Standard Mileage Reimbursement at $0.53 cents per mile
- Increased Mileage Reimbursement at $0.60 cents per mile for long distance clients
- Bi-Weekly Phone Stipend

Job SummaryOpportunity Community Services is a dedicated provider of Relocation Services (RSC) and Moving Home Minnesota (MHM) services. RSC and MHM strives to facilitate the transition of individuals from eligible institutions to community-based settings by offering comprehensive medical, social, educational, and other support services. Our mission is to create opportunities for Minnesotans to experience a seamless and person-centered shift from institutional living to community inclusion. We are seeking a highly motivated and compassionate individual to join our team as a Housing Case Manager. The successful candidate will play a crucial role in assisting individuals residing in eligible institutions in gaining access to necessary medical, social, and educational services, ultimately supporting their transition to community-based living.
Primary Responsibilities- Assist clients in locating resources and suitable housing in the community
- Perform outreach to the community and appropriate agencies
- Work with waiver Case Managers to provide updates to outside agencies
- Assesses and refers individuals, to appropriate community services and resources
- Support and participate with packing items and moving belongings to new location
- Support and assistance in accessing furniture and other household needs
- Conduct one-on-one visits at locations including their homes and offices
- Meet regularly in person with Clients to discuss progress, problems, and plans
- Take clients to tour potential housing opportunities
- Assist clients in completing housing application
- Maintain accountability for personal caseload
- Promote company initiatives through appropriate service provision
- Demonstrate and implement all work according to company policy and procedure
- Maintain accurate records
- Promote and maintain a work environment where all are respected and treated professionally and respectfully
- Attends work regularly and in a timely manner
- Represent the company in a professional manner
- Build positive professional relationships with community and service providers
- Develop relationships with individuals served and identify strengths & interests
- Advocate for client needs and resources
- Monitor client progress toward established service goals
- Work with the Disability Linkage Line to identify appropriate supports and services
- Coordinate, and collaborate with OCS departments and service providers
Skills- Skilled in dissecting complex scenarios, identifying areas for enhancement, and implementing strategies for improvement
- Adept at organizing and prioritizing substantial workloads efficiently, ensuring optimal task management
- Demonstrated capability to work independently
- Effective verbal and written communication skills
Minimum Qualifications:- 4-year degree in Human Services, or related field with two years of experience
- Knowledge of community resources
- Computer proficiency required
- Ability to be organized and manage time effectively
- Must possess a reliable vehicle, active vehicle insurance, and valid Driver’s license
- Experience with individuals with disabilities
- Possess effective organizational and communication skills
- Successful completion of background check administered by DHS and E-Verify
- Possess valid Driver license, reliable transportation, and active car insurance
