CN Guidance & Counseling Services in Tennessee seeks a qualified candidate to join the FACT Team, delivering comprehensive community-based treatment and support to individuals with severe mental illness. Qualifications

• Bachelor's degree in social work, psychology, rehabilitation counseling or related field required.
• One year experience in a mental health setting at the Master's level or two years at the Bachelor's level.
• Ability to assist FACT Team recipients with housing and benefits navigation.

Responsibilities

• Assist FACT Team recipients in obtaining and maintaining adequate housing.
• Understand concrete services such as Medicaid, Medicare, DSS and SPOA housing.
• Complete referrals and linkages to community benefits and services.
• Conduct quality improvement, utilization review and performance improvement activities.
• Plan and coordinate services in office or field settings to support case management.
• Share data to support partnered decision-making and inform clients of progress.
• Complete QI training and participate in refresher trainings as needed.
• Engage in agency trainings and NYS FACT model trainings.
• Foster data sharing to support client progress and decisions.
• Complete QI training during onboarding and participate in refresher trainings as needed.
• Cultivate an inclusive culture and support safe working environments.
• Adhere to safety protocols and maintain a safe workplace.
• Health and Safety awareness and related responsibilities.

Education

Job description

JOB DEFINITION:

The mission of the CN Guidance FACT Team is to deliver comprehensive and effective services to individuals who are diagnosed with severe mental illness and whose needs have not been well met by more traditional service delivery approaches. The FACT Team provides an integrated set of evidenced-based treatment and support services through a highly individualized approach. Services are provided in the community in places and situations where problems arise.

EDUCATIONAL REQUIREMENTS:

Bachelor's Degree in social work, psychology, rehabilitation counseling or related field required.

EXPERIENCE REQUIRED:

One year experience working in a mental health setting at the Masters or higher level or two years' experience working in a mental health setting at the Bachelors level.

DUTIES AND RESPONSIBILITIES:

1. Assist FACT Team recipients in obtaining and maintaining adequate housing
2. Must have an understanding of concrete services i.e. Medicaid, Medicare, DSS and SPOA housing.
3. Completes referrals and linkage to community benefits, organizations and institutions that will assist the client in completing the stated treatment plan goals.
4. Responsible for Quality Improvement, Utilization Review and Performance Improvement activities including but not limited to data collection, analysis of outcome trends, and identification of service provision, which needs improvement, completion of case records and the timely submission of required documentations.
5. Plan and coordinate all services, in the office or in the field, which support and supplement the rendering of case management and care coordination services to clients including but not limited to applying for and maintenance of benefits through the Department of Social Services, Social Security Administration, housing applications etc.
6. Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress.
7. Complete QI training during onboarding and participate in QI refresher training as needed.
8. Actively engage in agency wide trainings and work groups as well as NYS specific trainings on and for the FACT model.
