MetroPlusHealth in New York is seeking a Housing Taskforce Care Manager to support members experiencing homelessness, coordinating medical and behavioral health care while navigating housing transitions. The role combines telephonic outreach, in-field visits, and collaboration with providers to optimize care and reduce avoidable services.

The ideal candidate holds an LMSW or LCSW license, has at least 3 years in care management, and is proficient with common office software.

Qualifications

• Master’s degree in Social Work required.
• 3 years of care management experience in health care or managed care setting.
• Proficiency with computers and web-based applications (Word, Excel, Teams, PowerPoint, Outlook).
• Ability to travel within the service area, including home visits and provider visits.
• Significant experience working with clients around homelessness, addictions, mental health, entitlements, and housing.
• Valid LMSW or LCSW license in NYS preferred.

Responsabilités

• Assess and address clinical, psychosocial, financial, and environmental needs.
• Provide services to a diverse member population.
• Engage members to empower them to manage health and wellbeing.
• Support transitions from shelter to permanent housing.
• Develop a care plan with input from member, caregivers, and providers.
• Communicate plan of care to PCPs and update as needed.
• Maintain accurate clinical and non-clinical documentation.
• Travel for home visits and community outreach as required.
