Qualifications

• Associate degree in a health or social services related field or 2 years equivalent experience.
• Clearance through Idaho Department of Health and Welfare Criminal History background check.
• Demonstrated willingness to work with low-income and unhoused populations.
• Understanding barriers for people with behavioral health and substance misuse problems.
• Working knowledge of Boise area social service and housing resources.
• Ability to work as a team member.
• Good verbal and written communication skills.
• Reliable and courteous.

Responsibilities

• Assist program participants in accessing housing resources and/or empowering them in their housing stability process.
• Maintain a caseload of eligible participants and work to remove barriers to housing stability with clients.
• Complete a housing stability plan with each participant.
• Assist participants through each housing step: search, application, move-in and ongoing support.
• Advocate for participants with written ROI when needed.
• Maintain links with local social service agencies and housing authorities.
• Assist with paperwork for securing and maintaining housing.
• Document encounters promptly according to SOPs.
• Facilitate warm handoffs and referrals to clinic staff for additional services.
• Serve as a housing resource for clinic staff.
